Synopsis

The Ship broke away from her moorings on the River Trent, and made contact with two other vessels before grounding.

Action taken:

The Chief Inspector of Marine Accidents has made recommendations to the ship manager with reference to:

  • The size of mooring ropes used in relation to the deck equipment fitted

  • Crew awareness of the tidal conditions in ports visited

  • The premature release of deck cargo lashings, particularly when a berth move is expected

As a result of the observations of inspectors during their visits to the ship, the Chief Inspector made further recommendations with reference to:

  • The provision of safe access when alongside

  • The use of personal protective equipment by visitors during cargo operations
