Safety Bulletins 2004

Safety Bulletins are produced when urgent safety issues emerge during the course of an investigation and the Chief Inspector decides to disseminate this important information in advance of the main investigation report being produced.

The following Safety Bulletins were issued in 2004;

Accidental release of Vinyl Chloride Monomer from Gas Carrier while alongside at Runcorn, 10 August 2004. Issued August 2004
Quick release arrangements for the manual deployment of inflatable liferafts. Issued August 2004.
Near lethal use of CO2 onboard the fishing vessel Elegance, 30 miles north-west off the Shetland Islands on 30 January 2004.
