Key issues:

  • Late and inappropriate action taken to avoid contact with a buoy.
  • Risks associated with short tow ropes, inability to readily lengthen the ropes and lack of reserve of power of the pulling tug not fully realised.
  • The need to nominate a person to be in overall charge not identified.
  • Passage plan incomplete.
  • Issues relating to the maintenance of watertight integrity, use of lifejackets and the survey, maintenance and testing of tow release equipment.

Key issues:

  • Evidence indicates that the master was drunk and physically abusive to the OOW, he then fell asleep for 2 hours before the vessel grounded.
  • No lookouts posted and no watch alarm in operation.
  • AIS data was not used to monitor the vessel as it approached the Land’s End TSS.
  • The vessel was not directed into port for inspection after the accident.

By law accidents, including major injuries, must be reported to the MAIB by the quickest possible means. This is so they can be investigated immediately before vital evidence decays, is removed or is lost.
